Terms & Conditions For Provision Of Services 

From Buckinghamshire Medical 

To You ‘The Client’

  1. These are the terms and conditions upon which we (Buckinghamshire Medical Limited) supply services to you as a client of Buckinghamshire Medical, and which services are described on our website www.bucksmedical.co.uk .

  2. Please read these terms and conditions carefully prior to the use of the services provided by Buckinghamshire Medical. This is important as it details whom Buckinghamshire Medical is, how our services are provided to you, how you (the client) and we may change or end the contract, what steps to take if you are not satisfied with the services provided and other important and relevant information. Please do contact us if you have any questions related to these terms. 

  3. By booking/purchasing an appointment/service via our website (www.bucksmedical.co.uk) or telephone then you are agreeing to the Terms and Conditions.

  4. You agree not to use our services for medically inappropriate purposes.

  5. Following consultation with our doctors if your condition changes or if you have any concerns about the management of your condition then you must seek medical advice from us or another reputable and appropriately qualified healthcare professional. In the event of any adverse or unforeseen effects of any treatment, medication or healthcare product initiated or recommended by a healthcare professional at Buckinghamshire Medical you must seek immediate medical assistance.

  6. The list of services currently provided by Buckinghamshire Medical, as well as information pertaining to them, is listed on our website (www.bucksmedical.co.uk). If a service you require is not listed on our website then please do contact us to find out if we are able to offer it for you. 

  7. Services at Buckinghamshire Medical are provided by doctors who have the necessary qualifications and registrations.

  8. Our doctors will expertly assess you for the symptoms you describe and subsequently evaluate the treatment options available to you. Our expert clinicians will take into account your medical history, recommended national guidelines as well as your needs. This process will also involve your input as it is important for us at Buckinghamshire Medical to work with you to aim to meet your needs without any compromise to your health and wellbeing. In the instance that you require further evaluation, medical investigations or specialist treatment outside of the scope of the consulting doctor then they will make this clear to you and recommend onwards referral to a suitable healthcare professional.    

  9. In certain clinically relevant situations, it may be necessary for our doctors to communicate information you provide us with other third-party health care professionals (which includes your NHS GP and/or Specialist), but we shall only do so with your instruction and consent. An example which highlights a situation would be when a referral for an MRI scan is required, or when you would like to continue a prescription via the NHS.  

  10. Appointments for services provided by Buckinghamshire Medical can be made via our website, telephone or email. Appointments are subject to availability and made available only at the sole discretion of Buckinghamshire Medical. We cannot make any guarantee to the availability of any specific doctor at any time. 

  11. Fees for services provided by Buckinghamshire Medical are detailed on our website and the booking form.

  12. A deposit from yourself is necessary at the time of booking an appointment. If you are booking the appointment via our website then the specific fee is visible on the online booking form. If you are booking the appointment via telephone then our operator will take the payment details from you over the phone. Successful deposit payment is necessary to confirm the appointment booking. The remainder of the service fee (minus the deposit) will need to be paid in full immediately at the end of the appointment/service provided. Additional charges may apply in the event of late payments. 

  13. By booking an appointment for our services, you are expressing your intention to purchase our services. At the point of booking a deposit is payable, the payment of which confirms your appointment booking. If we are unable to accept/confirm your appointment booking then we will inform you of this and not charge you for any services.

  14. We understand that sometimes you may need to cancel the appointments/services you have booked with us. Please inform us at least 48 hours in advance if you need to amend or cancel your appointment. This is so that we can open up the booking times for others. Please note that for appointments/services cancelled within 48 hours of the booked time the deposit is non-refundable. For appointments/services cancelled within 24 hours of the booked time (as well as missed appointments), 100% of the fee will be charged.

  15. In order to be able to provide services from Buckinghamshire Medical to you as the client, we will need to take some information from you. These include personal details taken at the time of booking and during the consultations/appointments. Please read our Privacy policy on how this information is obtained, stored and utilised.

  17. We may contact you via phone, email or the postal address you provide us with. Provision of the aforementioned methods of contacts assumes consent from you for Buckinghamshire Medical to make contact with you. 

  18. Prior to the provision of services at Buckinghamshire Medical we will need to verify your identity. This will require two pieces of identification, one of which must be photographic and the other which includes your current address dated within the past three months.

  19. In order for us to provide you with safe medical care we will need full and accurate information about your medical history, medication history, allergy history, current symptoms and other clinically relevant information the doctor will ask of you. In the event that the doctor requires clinically relevant information (e.g. recent blood tests or scan results from another healthcare provider) then please ensure you bring these to the appointment. 

  20. If there is any clinically relevant information you would like the doctor to take into account the please bring verifiable evidence of this to your consultation.

  21. Please note that Buckinghamshire Medical or its healthcare professionals will not be responsible for providing the services late or not providing any part of them if this is caused by the client not giving us the information we need within a reasonable time of us requesting for it. 

  22. If you do not provide accurate clinically relevant information to us then this may impede with provision of safe, effective and appropriate care. Due to this, in certain cases, we may need to end the contact (with the full service fee payable) or make an additional reasonable fee to compensate for any resulting extra work necessitated.

  23. Prescriptions are issued at the sole discretion of the doctor only, and fees may apply. If the doctor decides to issue a prescription then a printed or emailed prescription will be provided to you. You may take this prescription to a pharmacy of your choice. The pharmacy will usually charge you a fee for the medication prescribed and any services charges related to this - Buckinghamshire Medical or its healthcare professionals are not liable for any fees charged by the pharmacy and are not liable for any delays or lack of supply of medications at the pharmacy of your choice.  

  24. Any delays outside the control of Buckinghamshire Medical are not its responsibility or liability. If an event outside the control of Buckinghamshire Medical occurs then we will contact you as soon as possible to inform you the event and the steps taken - as possible - to minimise the effect of the event. If there is a substantial delay from an event outside of the control of Buckinghamshire Medical then you may end the contract and received a refund for any services you have paid for but not fulfilled.  

  25. We may have to halt our services in order to reflect alterations in applicable laws or regulations, or to modify the services in response to your requests or our own notifications to you. These modifications could have an impact on your scheduled appointments. Unless it is an urgent or emergency situation, you will receive advance notice from us regarding the suspension of our services. In the event that we do suspend services and an appointment is delayed by more than four weeks, you have the option to contact us to terminate the contract.

  26. If you are experiencing complex medical symptoms that necessitate specialist care, we may need to refer you to a medical specialist or consultant who may not be affiliated with Buckinghamshire Medical. We do not hold responsibility or liability for any advice or treatment by an external medical practitioner, regardless of whether the referral originated from Buckinghamshire Medical

  27. We are responsible to you for foreseeable loss and damage caused by us. If we fail to comply with these terms, we are responsible for loss or damage you suffer that is a foreseeable result of our breaking this contract or our failing to use reasonable care and skill, but we are not responsible for any loss or damage that is not foreseeable. Loss or damage is foreseeable if either it is obvious that it will happen or if, at the time the contract was made, both we and you knew it might happen.  

  28. We do not exclude or limit in any way our liability to you where it would be unlawful to do so. This includes liability for death or personal injury caused by our negligence or the negligence of our employees, agents or subcontractors and for fraud or fraudulent misrepresentation.

  29. We may transfer this agreement to someone else. We may transfer our rights and obligations under these terms to another organisation. We will always tell you in writing if this happens and we will ensure that the transfer will not affect your rights under the contract. 

  30. You may only transfer your rights or obligations under these terms with our written consent. 

  31. Nobody else has any rights under this contract. This contract is between you and Buckinghamshire Medical. No other person or entity shall have any rights to enforce any of its terms. 

  32. If a court finds part of this contract illegal, the rest will continue in force. Each of the paragraphs of these terms operates separately. If any court or relevant authority decides that any of them are unlawful, the remaining paragraphs will remain in full force and effect. 

  33. Even if we delay in enforcing this contract, we can still enforce it later. If we do not insist immediately that you do anything you are required to do under these terms, or if we delay in taking steps against you in respect of your breaking this contract, that will not mean that you do not have to do those things or prevent us taking steps against you at a later date. For example, if you miss a payment and we do not chase you but we continue to provide the services, we can still require you to make the payment at a later date. 

  34. These terms are governed by English law and you can bring legal proceedings in respect of the services in the English courts. If you live in Scotland you can bring legal proceedings in respect of the services in either the Scottish or the English courts. If you live in Northern Ireland, you can bring legal proceedings in respect of the services in either the Northern Irish or the English courts.

  37. Any information provided on our website or other verified marketing material is for general information and educational purposes only, and it is not equivalent or intended to advice for a qualified and reputable doctor upon which you should rely.

  39. Buckinghamshire Medical is regulated by the Care Quality Commission (CQC). The CQC independently regulates health and social care provided in England.
